For your first consultation, please note that it may take a little longer as the doctor will review and document both your family and medical history.
To ensure you receive the best care possible, it’s essential for your doctor to have a complete and accurate record of your health.
- We kindly ask all new patients to complete a New Patient Registration form.
Please also complete the file transfer form so we can request your medical records from your previous GP. This helps us ensure we have all relevant information for your care.
To make the process smooth, we ask that you arrive 10 minutes early so we can collect your information and enter it into our system before your consultation.
